合法监护人

• If you are under 18 years of age , ask your parent or legal guardian to complete and sign section 4
  如你未满十八岁，你的父母或合法监护人须填写第四栏并签署。
• Those who are under 18 years of age , the applications must be signed by the parent or legal guardian
  若申请人的年未满十八岁，申请表必须由父母或合法监护人签署。
• Applications for children under 16 years of age must be signed by a parent or legal guardian
  年满11岁但未满16岁儿童的申请书，必须由一名父、母或合法监护人签署。
• Photostat copy of the consenting parent or legal guardian s identity card or travel document ; and
  同意这项申请的父亲或母亲或合法监护人的身份证或旅行证件影印本;及
• If the child is accompanied by a legal guardian , proof of guardianship is also required
  如儿童由合法监护人陪同申请，合法监护人还须出示可证明其监护人身份的文件。
• Photostat copy of the consenting parent or legal guardian s identity card or travel document ; and
  同意这项申请的父亲或母亲或合法监护人的身份证或旅行证件影印本;及
• The declaration at part 5 has to be made by your parent or legal guardian if you are aged under 18
  如你未满十八岁，须由你的父或母或合法监护人在第五部份作出声明。
• Applicants under the age of 16 must submit the application in person with a parent or legal guardian
  十六岁以下的儿童必须在父亲或母亲或合法监护人陪同下亲自递交申请
